Job Summary
Licensed Practical Nurse oversees a nursing team (CMTs/CNAs), administers medications and treatments per physician orders, conducts rounds, ensures adherence to nursing standards, mentors staff, documents resident care, coordinates with physicians, completes MDS-related activities, maintains medical records, and supports resident care conferences to tailor individualized care plans, with emphasis on holistic, patient-centered care and adherence to facility policies.
Required Qualifications
- Current MO LPN licensure or ability to obtain MO LPN licensure
- Must have Level I Medication Aide Training Program in the State of Missouri
- IV Certification or ability to obtain IV Certification
- Ability to supervise, mentor, and delegate to CNAs and CMTs
- Knowledge of nursing processes (assessment, planning, intervention, evaluation)
- Experience in geriatrics and care of chronically ill/convalescent adults
- Familiarity with MDS processes and facility policies
- Ability to document accurately and communicate with residents, families, and healthcare team
- Compliance with HR policies and procedures
