Job Summary
Licensed Practical Nurse (LPN) – administers medications, implements primary care orders, and evaluates participant complaints via in-person and telehealth within the PACE clinic; collaborates with the Interdisciplinary Team to plan and execute interventions per the participant's plan of care; educates participants and caregivers about medications and treatment instructions; maintains medical records and ensures charting and reporting requirements; participates in triage and assists in scheduling and coordinating care; adheres to BoldAge policies, OSHA and safety guidelines; requires graduation from an accredited nursing program and current LPN/LVN license; BLS required; IV certification preferred; 1 year of experience with frail/elderly population preferred; Monday–Friday, full-time at East Brunswick, NJ.
Required Qualifications
- Graduate of an accredited school of nursing
- Current LPN/LVN license in good standing to practice in the required state (NJ)
- BLS required (within 90 days of employment)
