Licensed Practical Nurse-4

Conduct monthly telephonic outreach to assigned Chronic Care Management patient panels, completing at least 20 minutes of non-face-to-face care management per patient. Assess patient status, reinforce individualized care plans, and support medication adherence while identifying barriers to care such as transportation or access issues. Document all interactions accurately in the electronic health record and escalate clinical concerns to providers per workflow standards. Provide education on chronic disease management, monitor for signs of deterioration, and coordinate services to reduce avoidable hospitalizations and emergency department utilization. Work 40 weekly hours as a regular employee with access to health insurance, retirement plans, and tuition reimbursement.
