Job Summary
Licensed Practical Nurse (LPN) needed for a Full-Time evening shift (2:00pm-10:00pm, Monday-Friday) in a post-acute care setting. Responsibilities include rendering nursing care per the nursing care plan, administering medications, documenting in the EMR, contributing to patient-focused care plans, leading team conferences on individualized care plans, assisting with emergencies, coordinating communication among patients, families, and the healthcare team, and providing health teaching as indicated. Must have strong documentation, communication, and collaboration skills; maintains infection control and HIPAA compliance; requires at least a High School diploma and completion of an accredited LPN program, plus an active LPN license and CPR certification.
Required Qualifications

- Graduate of an accredited LPN program
- Valid State Appropriate LPN license
- Current CPR certification
