Job Summary
Licensed Practical Nurse on an ACT team in Smithtown, NY, providing lead nursing duties, psychiatric and medical assessment, medication administration (injections/medications), and ongoing treatment planning. Responsibilities include assisting prescriber assessments, completing nursing assessments within set timelines, collaborating on psychiatric history and diagnoses, delivering individual/group/family therapy and education, documenting progress notes in EHR within 48 hours, maintaining medication administration records, monitoring vital signs, coordinating with physicians, dietitians, hospitals, and families, and using the agency vehicle to transport to appointments. Requires NYS LPN license, valid driver’s license, and ability to work flexible hours with potential 24/7 crisis coverage. Previous clinical supervisory experience, knowledge of ACT model, and strong communication skills are preferred.
Required Qualifications
