Licensed Practical Nurse - 1:1 Patient Care

Job Summary
Provide 1:1 in-home skilled nursing care per the provider's plan of care, administering medications, performing wound care, and monitoring patient vitals and respiratory status. Assess, monitor, and document patient condition while responding to complications such as infection or skin breakdown according to protocols. Partner with clients, families, and the care team to support goals of care, educate on routines and equipment, and coordinate supplies and ongoing care planning. Complete timely clinical notes and communicate shift handoff information to ensure continuity. Coordinate with the supervising RN and office team to ensure excellent outcomes and reliable coverage.
Required Qualifications
- Graduate of an accredited practical (vocational) nursing program
- Current LPN/LVN license in the state(s) in which you practice
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
- Able to visually and aurally observe and assess the patient
- Able to provide proof of valid driver's license and auto liability insurance if assignments include driving own vehicle to transport patient
- Able to frequently lift, reposition, and transfer patients
- Medium physical activity may require occasional lifting, pushing, or pulling up to 50 lbs
Desired Qualifications
- One year of clinical experience
- home care/private duty experience
- Trach and/or ventilator experience
