Licensed Practical Nurse - 1:1 Patient Care

Job Summary
Provide 1:1 in-home skilled nursing care per the provider's plan of care, administering medications, wound care, and specialized procedures like trach or ventilator support. Assess, monitor, and document patient vitals, blood glucose, and respiratory status while responding to signs of infection or distress. Partner with clients, families, and the care team to support goals of care, educate on routines, and coordinate supplies and equipment needs. Complete timely clinical notes and communicate shift handoff information to ensure continuity. Work consistent 1:1 shifts (8 or 12 hours, day or night) with no overtime required, offering daily pay and flexible scheduling.
Required Qualifications
- Graduate of an accredited practical (vocational) nursing program
- Current LPN/LVN license in the state(s) in which you practice
- One year of clinical experience
- home care/private duty experience
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
- Valid driver's license
- Auto liability insurance
- Able to visually and aurally observe and assess the patient
- Able to frequently lift, reposition, and transfer patients
- Medium physical activity may require occasional lifting, pushing, or pulling up to 50 lbs
Desired Qualifications
- One year of clinical experience preferred
- home care/private duty experience a plus
- Trach and/or ventilator experience
- Completion of the Office's preceptor program
