Job Summary
Conduct thorough evaluations to assess patients' physical conditions and identify functional limitations. Design personalized treatment plans that align with each resident's specific goals, implementing therapeutic exercises, manual therapy, and advanced modalities for pain relief and functional enhancement. Regularly document patient progress in accordance with facility protocols and engage in interdisciplinary meetings to discuss care strategies with physicians and nurses. Educate patients and families on effective home exercise programs to maintain progress outside of therapy sessions. Uphold compliance with state regulations and facility policies while collaborating with a multidisciplinary team to ensure comprehensive care for elderly residents. Monday to Friday, 9 AM to 5 PM onsite at the Rehabilitation Center.
Required Qualifications
- Licensed Physical Therapist
- Bachelor's or Master's degree in Physical Therapy from an accredited institution
- Doctor of Physical Therapy (DPT) degree
- Valid and unrestricted Physical Therapy license in the state of New York
- Strong clinical, communication, and interpersonal skills
- Proven ability to connect with patients and their families
- Compassionate and patient-centered care philosophy
- At least two years of experience working as a Physical Therapist
- Experience in skilled nursing facilities or rehabilitation settings
