Job Summary
Conduct thorough evaluations for an average of 7 patients daily, identifying physical therapy needs and setting personalized treatment goals. Design and implement effective treatment plans utilizing evidence-based practices, while educating patients and families on processes and resources. Facilitate progress tracking and documentation through advanced electronic health record systems to ensure compliance and team communication. Collaborate seamlessly with interdisciplinary teams including occupational therapists, speech therapists, and medical staff to ensure comprehensive care for patients recovering from surgery, injury, or chronic conditions. Work onsite Monday through Friday from 9:00 AM to 5:00 PM within a modern rehabilitation facility.
Required Qualifications
- Current New York State Physical Therapist License
- Bachelor's Degree in Physical Therapy or a related field from an accredited institution
- Minimum of 2 years of relevant clinical experience in a rehabilitation setting
Desired Qualifications
- Experience working in a Skilled Nursing Facility or similar rehabilitation environment
- Familiarity with patient management software and electronic health records (EHR) systems
- Certification in specialized areas of physical therapy, such as orthopedics or geriatrics
