Job Summary
Conduct comprehensive evaluations and assessments of patients in their home settings to determine effective treatment strategies. Develop individualized treatment plans incorporating therapeutic exercises, mobility training, and manual therapy aligned with recovery goals. Document all treatments, responses, and progress meticulously while continuously monitoring patient outcomes to adjust plans as necessary. Educate patients and families on rehabilitation strategies and mentor team members on techniques and assistive equipment usage. Required Qualifications
- A Bachelor's degree in Physical Therapy or Doctorate of Physical Therapy (DPT) from an accredited institution.
- A valid Physical Therapy license in the state of Florida.
- Proficient verbal and written communication skills, with the ability to interact effectively with patients and interdisciplinary team members.
- Strong organizational skills with a commitment to maintaining thorough patient records.
- A compassionate and patient-centered approach to care.
- Highly adaptable with the ability to work collaboratively in a dynamic environment.
- Must be able to travel within Lehigh Acres and surrounding areas
- Must be available for flexible scheduling options from Monday to Friday
Desired Qualifications
- At least 2 years of experience in a clinical physical therapy setting, with preference given to those familiar with home care practices.
- Experience with a wide range of therapeutic modalities and techniques.
- Working knowledge of electronic health record (EHR) systems.
