Job Summary
Conduct thorough assessments of physical health needs and functional limitations to ensure accurate diagnoses and effective treatment plans. Deliver tailored physical rehabilitation therapies using modalities such as manual therapy, therapeutic exercises, and patient education. Monitor patient progress, adapt treatment plans, and instruct patients on independent home exercises. Facilitate communication between patients, families, and multidisciplinary team members while maintaining accurate documentation. Participate in regular case reviews and team meetings. Work onsite Monday to Friday from 9:00 AM to 5:00 PM with some scheduling flexibility. This role supports a diverse patient population recovering from injuries, surgeries, or chronic conditions within a collaborative outpatient psychiatric facility environment.
Required Qualifications
- Licensed Physical Therapist
- Bachelor's degree in Physical Therapy from an accredited institution
- relevant licensure to practice in California
- Comprehensive understanding of physical rehabilitation techniques

- ability to build rapport and trust with patients and their families
- Strong organizational and time management abilities
- Familiarity with electronic health record (EHR) systems for documentation and patient tracking
Desired Qualifications
- A minimum of 2 years of clinical experience in a rehabilitation setting
- Experience working with a psychiatric population or integrated behavioral health initiatives
- Previous involvement with evidence-based physical therapy methodologies and treatment frameworks
- Knowledge of community resources for patient referrals and support services
