Job Summary
Conduct thorough patient evaluations, including medical history reviews and specific assessments, to establish tailored treatment plans. Communicate findings clearly to patients and families while implementing treatment protocols that adapt to evolving healthcare landscapes. Oversee discharge planning, ensure compliance with local, state, and federal regulations, and maintain accurate documentation of care activities and progress. Consult with multidisciplinary teams as needed and manage time effectively by meeting productivity expectations and attendance requirements. Deliver compassionate, patient-centered care to individuals recovering from spine and orthopaedic injuries, surgical interventions, chronic pain, and mobility challenges in a home care setting.
Required Qualifications
- Bachelor's Degree in Physical Therapy from an accredited program
- Valid state license to practice
- Strong communication skills, both verbal and written
