Job Summary
Conduct thorough examinations to determine patient functional capabilities and create tailored treatment plans. Implement regimens while re-evaluating progress, modifying plans as necessary, and providing education on exercises to patients and families. Consult with healthcare providers to align on care pathways and maintain accurate documentation in compliance with regulations. Guide less experienced staff and students, manage departmental resources, and participate in community outreach activities. Provide comprehensive discharge planning to ensure smooth patient transitions. This role involves full-time onsite work at a comprehensive rehabilitation center in West Hartford, CT, serving diverse populations from post-operative recovery to chronic pain management.
Required Qualifications
- Bachelor's Degree in Physical Therapy
- Current state licensure in Physical Therapy
- Basic Life Support (BLS) certification
- Strong communication skills, both verbal and written
- Proficiency in basic computer skills and word processing
