Conduct thorough evaluations of patients with functional limitations by gathering comprehensive history and performing specific assessments. Develop and implement individualized treatment plans that align with evaluation findings, modifying protocols based on ongoing assessments. Utilize a variety of treatment modalities and therapeutic interventions tailored to patient needs, including those recovering from orthopedic injuries, managing chronic conditions, or post-surgery. Collaborate effectively with interdisciplinary team members to inform them of changes in patient conditions requiring care plan modifications. Maintain accurate and timely documentation of patient interactions, treatment protocols, progress notes, and discharge summaries. Adhere to state practice acts and regulations governing physical therapy practice. Participate in training and educational sessions to stay current with clinical best practices and compliance requirements. Work Monday to Friday from 8:00 AM to 5:00 PM in a warm, inclusive outpatient setting focused on evidence-based care and patient satisfaction.
