Job Summary
Develop and implement personalized physical therapy plans to enhance mobility, reduce pain, and prevent disability based on physician prescriptions. Conduct thorough diagnostic assessments of clients' physical conditions, monitor progress meticulously, and adapt care plans as necessary. Assist clients in performing therapeutic exercises while educating them and families on proper techniques and safety. Evaluate home environments for modifications and participate in interdisciplinary meetings to adjust care plans and foster communication with physicians, nurses, and case managers. Ensure compliance with documentation requirements and maintain timely updates on treatment plans. This full-time onsite role in Orland Park requires an Illinois-licensed Physical Therapist with at least one year of experience, offering a salary of USD 100,000 to 120,000 annually.
Required Qualifications
- Graduate of an accredited physical therapy program with a Bachelor's degree
- A minimum of one (1) year of experience in physical therapy, preferably in home care or acute care settings
- Current physical therapy licensure in good standing in the state of Illinois
- Valid CPR certification
- A driver's license with proof of automobile insurance
