Job Summary
Conduct comprehensive physical assessments for patients in their homes, analyzing unique needs to establish personalized treatment plans. Educate patients and families on home exercises and preventative measures while documenting all interactions to ensure compliance and monitor progress. Collaborate with interdisciplinary teams to coordinate holistic care for older adults, those with chronic pain, and individuals recovering from injuries. Maintain continuous professional development through relevant training and workshops.
Required Qualifications
- A valid and active state Physical Therapist license in California
- A Bachelor's degree in Physical Therapy from an accredited institution
- Demonstrated expertise in assessing and treating a wide variety of conditions affecting mobility and functionality
- Strong communication and interpersonal skills
- Proficiency in developing individualized therapy plans and modifying them based on ongoing assessment and patient progress
- Knowledge of legal and ethical considerations in the delivery of home health care
