Job Summary
Deliver personalized rehabilitative therapy to residents in a 176-bed skilled nursing environment. Guide patients through tailored exercises to enhance mobility, balance, and strength. Track and document progress to inform care plans. Educate patients and their families on therapy goals and home exercises. Collaborate with an interdisciplinary healthcare team. Maintain a safe, clean, and supportive treatment environment. Adhere to healthcare and safety standards. Stay current through ongoing training and professional development.
Required Qualifications
- Active Colorado Physical Therapist license (required)
- Degree from an accredited Physical Therapy program (Including Associate‘s Degree)
- 1+ year of experience preferred (new grads welcome to apply)
- Strong communication, attention to detail, and documentation skills
- Ability to work autonomously and collaboratively
- Physical ability to support patient mobility and lift equipment repetitively
