Provide skilled therapy services under the direction of a licensed Physical Therapist in a Skilled Nursing Facility setting. Assist in developing and implementing individualized treatment plans while documenting patient progress using EMR systems. Foster strong communication with PTs, OTs, SLPs, and nursing staff to promote patient independence and mobility. This role supports a collaborative, patient-focused rehab team in Freehold, NJ, offering mentorship and continuing education opportunities for new grads and experienced professionals.
Required Qualifications
- Graduate of an accredited Physical Therapist Assistant program
- Current NJ license (or eligibility to obtain)
