Job Summary
Licensed Physical Therapist Assistant delivering home-health physical therapy under supervising PTs. Responsibilities include delivering therapeutic exercises and functional training to restore mobility, applying physical therapy modalities to reduce pain and improve function, monitoring and documenting patient progress, teaching patients and families to use assistive devices and follow home exercise programs, and collaborating with PTs, nurses, aides, and the care team to ensure high-quality, patient-centered care.
