Licensed P/V Nurse Continuous Care

Job Summary
Provide skilled bedside nursing care to hospice patients in facility or home settings, monitoring conditions, managing acute symptoms, and administering medications. Assist with plan of care preparation, infection control, and symptom management while documenting activities and communicating with interdisciplinary teams. Participate in call schedules and provide on-call nursing support as directed. This role requires an LPN/LVN licensed in the state of employment with at least two years of experience in hospice, home health, or long-term care. Travel is required between patient locations, and candidates must hold a valid driver's license and current automobile insurance.
Required Qualifications
- Licensed Practical/Vocational Nurse (LPN/LVN) licensed in the state of employment
- CPR certification
- Valid driver's license and current automobile insurance
- Graduate of an accredited Practical Nursing program
- Minimum of 2 years' experience as an LPN/LVN in hospice, home health, or long-term care
- Strong knowledge of hospice patient needs, terminal illness care, and interdisciplinary team roles
- Excellent patient assessment and communication skills
- Understanding of regulatory guidelines such as Medicare, Medicaid, and ACHC
- Ability to provide compassionate support to patients and families during end-of-life care
- Effective documentation and program management skills
- Compassionate and sensitive to the emotional needs of terminally ill patients and their families
- Flexible and adaptable, able to manage multiple tasks efficiently
- Professional, mature, and cooperative with strong interpersonal skills
- Proficient with nursing assessment tools, computers, fax, copier, and communication devices
- Reliable transportation required for travel between patient locations
- This position requires a Florida Level 2 background screening through the Care Provider Background Screening Clearinghouse
