Licensed Outpatient Therapist

Job Summary
Provide individual and family therapy to assigned consumers in outpatient or school-based mental health settings. Conduct psychosocial assessments at intake and during ongoing intervals, develop individualized treatment plans in accordance with county/state/federal requirements, and deliver therapy in recommended settings. Collaborate with the treatment team to address the whole-person needs, monitor and document progress through required clinical notes and discharge summaries, and participate in supervision and ongoing professional development. Responsible for maintaining an electronic health record, ensuring timely completion of progress notes and treatment reviews, and coordinating care with external referrals and supports. May travel between appointment sites as program requirements dictate (up to 100% travel).
Required Qualifications
- Bachelor’s degree from an accredited college or university in Psychology, Social Work, nursing, rehabilitation or activity program
- Or has a graduate degree in a clinical discipline with 12 graduate level credits in mental health or counseling
- Bachelor’s/Master’s level (Mental Health Worker) credentialing expectations are described in the job posting
