Job Summary
Comprehensively examine, evaluate, and develop tailored treatment plans for patients presenting with functional limitations and disabilities. Perform thorough evaluations including patient history reviews and specific assessments to formulate actionable recommendations. Demonstrate expertise in implementing treatment protocols that align with evaluation findings and evolving healthcare demands. Collaborate with a multidisciplinary team to modify patient care plans and document significant changes. Engage in appropriate discharge planning and follow-up care, ensuring timely and accurate documentation of all patient care transactions. Stay compliant with all corporate and clinical training requirements while adhering to local, state, and federal regulations. Take part in regular facility meetings and contribute to initiatives aimed at improving patient access and quality of care through Continuous Quality Improvement projects. Work Monday through Friday from 8:00 AM to 5:00 PM.
Required Qualifications
- A Doctorate in Physical Therapy (DPT) from an accredited program.
- Current state licensure as a Physical Therapist (PT) issued by the Physical Therapy Examining Committee.
- A minimum of two years of experience in an outpatient orthopedic setting, demonstrating a strong foundation in clinical techniques and patient care.
- Valid current CPR Certification.

- Experience with evidence-based practices and patient-centered care approaches.
- Familiarity with electronic medical record (EMR) systems that facilitate clinical documentation and patient tracking.
- Participation in multidisciplinary teams contributing to collaborative care strategies.
