Job Summary

Receive and carry out daily patient assignments under the direct supervision of a registered nurse or licensed therapist. Collect patient and family data, report findings, and perform interventions and procedures to ensure safe, effective quality of care. Execute a variety of patient care tasks including activities of daily living, vital signs collection, and ambulation. Serve as a Personal Care Attendant as needed while effectively communicating clinical information verbally and in writing to patients, representatives, and home care staff. Requires a valid Vermont state LNA license, high school diploma, and current driver's license with proof of auto insurance. Full-time position with up to $1,000 sign-on bonus and benefits including health, dental, vision, and 401(k) match.

Required Qualifications

  • High School diploma or equivalent
  • Successful completion of an accredited LNA program
  • Valid Vermont state LNA license
  • Valid state driver's license
  • Vehicle and automobile insurance
  • Ability to successfully pass pre-employment driver record, criminal, adult and child abuse background check/review
  • Proof of auto insurance at time of employment

  • Prior home care and/or hospice experience
