Job Summary
Diagnose and treat mental health disorders through personalized psychotherapy, helping clients define goals and gain insights across the general spectrum of conditions. Collaborate with an integrated care team to coordinate psychiatric and therapeutic services, including crisis intervention when needed. Conduct comprehensive assessments and develop individualized treatment plans while utilizing an EMR system to ensure accurate electronic health records. Partner with providers to move patients up and down the care continuum using a measurement-based approach that tracks outcomes to inform care plans. Work three days onsite and two days telehealth, with flexibility to design your own schedule in advance.
Required Qualifications
- Fully Licensed Therapist (LCSW, LPC, or LMFT) in the state of Wisconsin
- Experience conducting comprehensive assessments of clients and developing individualized treatment plans
- Proficiency in using an EMR system and ensuring accurate and up to date electronic health records for all patient encounters
- Understanding of mental health disorders and treatment modalities
- Desire and ability to see a general spectrum of mental health conditions
- Must be able to work three days a week onsite and two days a week telehealth
- Must be available for full-time hours
- Must be able to create your own work schedule in advance
