Job Summary
Independently Licensed Mental Health Therapist (LCSW/LMFT/LPC) to join a SNF therapy team in Albuquerque, NM. Responsibilities include completing diagnostic evaluations, providing psychotherapy and treatment planning, and collaborating with psychiatrists, psychologists, care coordinators, and facility staff for holistic geriatric care. Conduct documentation remotely in Athena EMR, ensure compliance with agency policies, and maintain strong interpersonal skills, flexibility, and organized workflow; Spanish fluency is a plus.
Required Qualifications
- Master of Social Work, Counseling, or other eligible master's degree and New Mexico licensure as an LCSW, LMFT, or LPC
- NPI (National Provider Identifier)
- Credentialed with Medicaid/Medicare a PLUS!
- TB Test
- CPR/AED/First Aid Certification
- Fluency in Spanish is a plus
