Licensed Mental Health Therapist

Provide evidence-based outpatient therapy, conduct comprehensive psychosocial assessments, and develop individualized treatment plans for diverse patient populations. Collaborate closely with psychiatrists, primary care providers, and care managers to coordinate holistic care, ensure continuity of services, and support crisis intervention. Maintain accurate clinical documentation in the EMR, participate in case conferences, and contribute to quality-improvement initiatives and patient education programs. This role supports Kaiser Permanente's Healthcare & Medical Services department by delivering integrated mental health care across outpatient settings.
