Job Summary
Diagnose and treat mental health disorders through psychotherapy while collaborating with healthcare professionals for care coordination and crisis intervention. Conduct comprehensive assessments to develop individualized treatment plans and utilize a measurement-based care approach to track outcomes. Manage patient case loads across a hybrid schedule of three days onsite and two days telehealth, with flexibility to design your own work hours. Partner with the integrated care team to move patients up and down the care continuum as needed.
Required Qualifications
- Active license (LCSW, LPC or LMFT) in the state of Texas
- Experience conducting comprehensive assessments of clients and developing individualized treatment plans
- Proficiency in using an EMR system and ensuring accurate and up to date electronic health records for all patient encounters
- Understanding of mental health disorders and treatment modalities
- Desire and ability to see a general spectrum of mental health conditions
- Must be able to work three days a week onsite and two days a week telehealth
- Must be available for full-time hours
- Must be able to provide crisis intervention if needed
