Licensed Mental Health Provider- PRN

Licensed Mental Health Provider (LSCSW/LCPC) needed PRN for nursing home settings in Oklahoma. Responsible for mental health counseling, diagnostic evaluation and assessment, and related services within an integrated primary care behavioral health model. Services may be delivered in telehealth remote, in-clinic, and designated facilities with partial in-clinic duties possible. Responsibilities include functional assessments (bio-psych-social), planning and implementing interventions with patients and families, providing evaluation/diagnosis and targeted interventions, maintaining encounter notes and clinical documentation (intake assessments, progress notes, treatment plans, discharge notes), providing information and referral services, collaboration with referral sources and team members, coordinating care with facilities when applicable, and meeting productivity expectations. Requires a Master’s degree in a relevant field and an active/unrestricted license with at least 2 years of supervised experience. Proficiency in English, strong interpersonal skills, ability to escort/transport patients, and ability to work all shifts. Safety-sensitive position.
