Job Summary
Provide individual and group psychotherapy to geriatric residents in subacute or skilled nursing facilities; assess, diagnose, and treat emotional and behavioral health conditions (including depression, anxiety, adjustment disorders, and cognitive decline); develop and implement evidence-based treatment plans; collaborate with facility staff and family members; maintain accurate clinical documentation in an EHR; monitor patient progress and adjust treatment plans as needed; role supports either full-time or part-time with 32-40 hour weekly equivalents; cannot be overloaded with administrative tasks; includes benefits such as medical/dental/vision, short/long-term disability, 401(k) match, malpractice coverage, PTO/holidays, and sign-on bonus for FT; opportunities for mentorship, CEU reimbursement, and clinician-designed EHR system.
Required Qualifications
- Active and unrestricted license to practice as a Licensed Mental Health Counselor in State applying for
- English communication
- Proficiency with electronic health records (EHR)
- Ability to provide in-person services at assigned facilities up to 45 minutes
- Strong organizational and documentation skills
