Job Summary
PRN Licensed Medical Social Worker provides psychosocial assessments, counseling, and referrals to hospice patients across the San Antonio area, develops and evaluates plans of care in collaboration with patients and families, and collaborates with an interdisciplinary team to support comprehensive patient care. Responsibilities include maintaining accurate documentation, contributing to QAPI initiatives, staying current with eligibility standards, and promoting the organization’s policies. Requires an MSW and active LMSW/LICSW/LCSW licensure, CPR certification, and reliable transportation. Compensation is $65 per visit; benefits available for full-time positions as listed.
Required Qualifications
- Master's or Doctoral Degree from an accredited school of Social Work
- Active social work licensure (LMSW, LICSW, or LCSW) in the state of practice
- Minimum of one year experience as an MSW in a healthcare setting
- Proof of current CPR certification
- Reliable transportation with a valid driver's license and auto liability insurance
