Job Summary
Collaborate with the Goals of Care team to develop robust programs for Interim clients, aiming to improve satisfaction and facilitate hospice transitions. Assess psychosocial status and deliver care aligned with the physician's plan of care while addressing social, ethical, and emotional factors. Effectively communicate with the interdisciplinary healthcare team to coordinate patient care during transitions between home, hospital, and post-acute facilities. Work as part of a patient-centric home health team supporting full-range services with integrity.
Required Qualifications
- Master of Social Work (MSW) degree
- MSW in the state in which the position is located
- Minimum 3 years of social work experience in a health care setting
