Job Summary
Conduct psychosocial assessments and develop individualized care plans for patients and families in Denver, CO. Provide counseling, crisis intervention, and resource navigation while supporting care coordination, advance directives, and long-term planning. Document care provided and collaborate with interdisciplinary teams to serve as an advocate and liaison between patients, families, and community services. Earn $75–$90 per completed visit with mileage reimbursement, working flexible Monday–Friday hours of 1–5 per week. Requires an active Colorado MSW license, one year healthcare experience, CPR certification, and a licensed driver's license.
Required Qualifications
- Active MSW license with the state of Colorado
- Experience working with both adults and peds/family dynamics
- Minimum of one year's experience in health care
- Current CPR certification
- Licensed driver with an insured automobile in good working order
