Job Summary
Conduct in-person initial assessments for PACE participant enrollment to obtain psycho-social histories, including cognitive status, social supports, and mental health needs. Collaborate with interdisciplinary teams to develop comprehensive care plans and perform quarterly reassessments or as conditions change. Provide ongoing counseling, education, and advocacy regarding aging, dementia, and end-of-life care while maintaining participant housing through interventions and referrals. Perform home visits to assess living environments and coordinate hospital admissions and discharges within 24 hours of admission. Act as a liaison with community agencies, mediate care discussions, and facilitate end-of-life planning or facility placements. Complete all clinical documentation and assist participants with Medicaid eligibility, Social Security applications, and legal directives. Serve as part of a rotating on-call pool for support outside regular hours.
Required Qualifications
- Must be at least 18 years of age
- Master's degree in social work from an accredited school of social work
- Active LMSW in the state of Kansas
- Current driver's license and proof of auto insurance
- Working knowledge of Microsoft Office Suite
- Must have working knowledge of Microsoft Office Suite
- Must be able to lift up to 20 pounds of force occasionally
- Must be able to lift up to 10 pounds of force frequently
- Must be able to walk or stand to a significant degree
- Must be able to sit most of the time
- Must be able to push and/or pull of arm or leg controls
Desired Qualifications
- Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable
