Job Summary
Conduct surveillance and investigations at nursing homes, transitional care units, and adult day health programs to evaluate compliance with federal and state regulations. Review allegations of abuse and neglect, perform interviews with facility staff, residents, and families, and draft Statements of Deficiencies within required timeframes. Assess compliance with Plans of Correction, complete data entry in federal and state reporting databases, and participate in emergency surveys during immediate jeopardy situations. Testify in administrative hearings as needed and adhere to Department of Health work schedules, including rotating weekend, holiday, and evening shifts. This role offers full-time employment with on-site travel requirements and premium pay for holidays and overtime.
Required Qualifications
- Master's degree in Social Work (MSW) from an accredited program
- Current New York State Licensed Master Social Worker (LMSW)
- Minimum two (2) years of clinical experience in a medical facility
- Two (2) years of experience performing nursing home surveillance activities
- Ability to obtain Surveyor Minimum Qualifications Test (SMQT) certification within 12 months of employment
- Ability to travel to an on-site location
- Candidates must be flexible with scheduling
- Employees must be available to work weekends, holidays, and evening shifts, as needed
- Weekend, holiday, and evening shift schedules will be assigned on a rotating basis
- This is a travel position
