Job Summary
Conduct comprehensive psychosocial assessments, evaluate social and emotional needs, and develop individualized care plans for patients and families experiencing illness, disability, end-of-life care, and bereavement. Provide counseling, crisis intervention, emotional support, and bereavement services while coordinating community resources and facilitating advance care planning. Collaborate with physicians, nurses, and therapists to ensure high-quality patient-centered care, maintain accurate clinical documentation, and ensure compliance with federal, state, Medicare, and hospice regulations. Some evening or weekend visits may be required.
Required Qualifications
- Master's Degree in Social Work (MSW) from a program accredited by the Council on Social Work Education (CSWE)
- Current Licensed Master Social Worker (LMSW) license in the State of Texas
- Current driver's license and reliable transportation
- Current automobile liability insurance
- Ability to travel to patient homes, facilities, and community locations
- Ability to sit, stand, walk, bend, and lift up to 25 pounds occasionally
- Ability to utilize a computer, tablet, and other office equipment
- Some evening or weekend visits may be required
Desired Qualifications
- Minimum one (1) year of social work experience
- Experience in Home Health, Hospice, Healthcare, Case Management, Geriatrics, Palliative Care, or Medical Social Work
- Knowledge of Medicare, Medicaid, and community resources
- Knowledge of hospice philosophy and end-of-life care principles
