Job Summary
Licensed Master Social Worker (LMSW) – Part-Time / PRN for Home Health & Hospice. Provides psychosocial assessments, counseling, resource coordination, bereavement support, and advocacy to patients and families experiencing illness, disability, end-of-life care, and bereavement. Collaborates with physicians, nurses, therapists, and the interdisciplinary team to develop and implement individualized care plans, facilitate discharge planning, and ensure HIPAA-compliant documentation. Requires travel to patient homes and community locations; some evenings or weekend visits may be required; flexible Part-Time/PRN schedule. Familiarity with Medicare/Medicaid and community resources is preferred; strong assessment, communication, and crisis intervention skills are essential.
Required Qualifications
- Master's Degree in Social Work (MSW) from a CSWE-accredited program
- Current Licensed Master Social Worker (LMSW) license in the State of Texas
- Current driver's license and reliable transportation
- Current automobile liability insurance
