Job Summary
Licensed Master Social Worker (LMSW) to join Home Health & Hospice team on a Part-Time/PRN basis. Responsibilities include psychosocial assessments of patients and families, counseling, crisis intervention, care-plan development with interdisciplinary team, resource coordination, bereavement support, discharge planning, and education on healthcare and community services. Must hold MSW from CSWE-accredited program and Texas LMSW license; travel to patient homes/locations required; field-based role with flexible Part-Time schedule including some evenings/weekends.
Required Qualifications
- Master's Degree in Social Work (MSW) from CSWE-accredited program
- Current Licensed Master Social Worker (LMSW) license in Texas
- Current driver's license and reliable transportation
- Current automobile liability insurance
- Minimum one year of social work experience preferred
- Experience in Home Health, Hospice, Healthcare, Case Management, Geriatrics, Palliative Care, or Medical Social Work preferred
