Job Summary
Licensed Master Social Worker (LMSW) to join Home Health & Hospice team on a Part-Time/PRN basis. Provide psychosocial assessments, counseling, resource coordination, bereavement support, and advocacy to patients and families experiencing illness, end-of-life care, or bereavement. Collaborate with physicians, nurses, therapists, and the interdisciplinary team to develop individualized care plans, facilitate advance care planning, coordinate discharge planning, educate families on available services, and ensure HIPAA-compliant documentation. Travel to patient homes or community locations for field-based visits; flexible schedule with some evenings or weekends as needed.
Required Qualifications
- Master's Degree in Social Work (MSW) from CSWE-accredited program
- Current Licensed Master Social Worker (LMSW) license in the State of Texas
- Current driver's license
- Reliable transportation
- Experience in Home Health, Hospice, Healthcare, Case Management, Geriatrics, Palliative Care, or Medical Social Work preferred
