Job Summary
Conduct comprehensive psychosocial assessments and evaluate social, emotional, and financial needs for patients and families experiencing illness, disability, or end-of-life care. Develop individualized care plans, provide counseling and crisis intervention, and coordinate resources including financial assistance and community services. Facilitate advance care planning, participate in interdisciplinary team meetings, and maintain accurate clinical documentation while ensuring compliance with federal, state, and Medicare regulations. Part-time and PRN positions available with flexible scheduling; some evening or weekend visits may be required. Candidates must hold a current Texas LMSW license and a Master's Degree in Social Work.
Required Qualifications
- Master's Degree in Social Work (MSW) from a program accredited by the Council on Social Work Education (CSWE)
- Current Licensed Master Social Worker (LMSW) license in the State of Texas
- Current driver's license and reliable transportation
- Current automobile liability insurance
- Ability to travel to patient homes, facilities, and community locations
- Ability to sit, stand, walk, bend, and lift up to 25 pounds occasionally
- Ability to utilize a computer, tablet, and other office equipment
- Some evening or weekend visits may be required
Desired Qualifications
- Minimum one (1) year of social work experience
- Experience in Home Health, Hospice, Healthcare, Case Management, Geriatrics, Palliative Care, or Medical Social Work
- Knowledge of Medicare, Medicaid, and community resources
- Knowledge of hospice philosophy and end-of-life care principles
