Job Summary

Role involves conducting psychosocial assessments, providing supportive counseling, developing and implementing patient care plans with the clinical team, case management, and ensuring documentation in electronic health records. Required Qualifications
- Education: Master of Social Work (MSW) from an accredited program
- Licensure: Active New York State license as a Licensed Master Social Worker (LMSW) (Required)
- Must reside in the tri-state area
- Experience: Minimum of 1 year of relevant experience
- Language: Must be fluent in Spanish
- Technical Skills: Proficiency in psychosocial assessments, knowledge of therapeutic modalities, case management, and electronic health records
- Soft Skills: Empathetic, culturally competent, excellent communication and collaboration skills, strong organizational abilities, and a commitment to ethical practice
