Job Summary
Conduct psychosocial assessments and provide supportive counseling under supervision. Collaborate with the clinical team to develop and implement patient care plans. Provide case management services and connect patients with necessary community resources while working closely with physicians, nurses, and other providers. Maintain accurate documentation of patient interactions and participate in team meetings for a coordinated care approach. This hybrid role in a multi-specialty practice in the Bronx requires an LMSW with at least one year of experience and fluency in Spanish.
Required Qualifications
- Master of Social Work (MSW) from an accredited program
- Active New York State license as a Licensed Master Social Worker (LMSW)
- Residing in the tri-state area
- Minimum of 1 year of relevant experience
- Fluency in Spanish
- Proficiency in psychosocial assessments
- Knowledge of therapeutic modalities
