Job Summary
Licensed Master Social Worker to provide Independent Living Skills Training Services (ILST) for waiver participants. Responsibilities include conducting comprehensive functional assessments of ADL/IADL, developing individualized ILST Detailed Plans with goals and milestones, and providing hands-on training in self-care, medication management, task completion, communication and interpersonal skills. Supports participants in mobility, community transportation, money management, and household maintenance; assess and address maladaptive behaviors and provide problem-solving skills training. Create a positive, supportive learning environment and ensure ILST services are delivered in the most effective setting (home or community). Qualifications include LMSW certification, knowledge of ADL/IADL assessment and training, strong interpersonal and communication skills, empathy, patience, and flexibility to adapt training approaches.
Required Qualifications
