Job Summary
Conduct biopsychosocial assessments per referral and policy, then carry out appropriate interventions for patients. Provide ongoing psychotherapeutic counseling and concrete services to patients, families, and significant others at assigned frequencies. Complete required documentation in a timely fashion and collaborate with families, collaterals, and providers on treatment, referrals, and aftercare planning. Escort patients into the community, make home visits as needed, and attend rounds with multidisciplinary teams. Advocate with social and community agencies to ensure patient services. Incorporate universal abuse screening and adhere to NYS mandated reporting protocols for child, elder, and intimate partner violence. Participate in performance improvement projects, regular supervision, and ad-hoc escalation as necessary. Comply with all hospital regulations, essential worker requirements, and dress guidelines.
Required Qualifications
- Licensed Master Social Worker
- Valid Licensed Master of Social Work (LMSW), license issued by NYSED, Primary Source Verification required
- One-year LMSW experience
- Outpatient Cancer Center
- Master's Degree
- Malpractice Insurance - All clinical workers are required to have active individual Medical Malpractice
- Adhere to company guidelines regarding COVID compliance
- Schedule Notes: 9:00 AM – 5:00 PM
