Job Summary
Certified Rehabilitation Counselor specializing in Independent Living Skills Training Services (ILST) conducts functional assessments of waiver participants, develops ILST Detailed Plans with goals and milestones, and provides hands-on training in self-care, medication management, task completion, communication and interpersonal skills, mobility, transportation, money management, and household maintenance. Services are delivered in participants' homes or community settings, with a focus on ADL/IADL-related skills and addressing maladaptive behaviors to maximize learning potential within Medicaid waiver programs (e.g., NHTD, TBI).
Required Qualifications
- Certification as a Licensed Master Social Worker
- Knowledge of ADL and IADL assessment and training
- Strong interpersonal and communication skills
- Empathy and patience in working with individuals facing disabilities
- Problem-solving skills and the ability to adapt training approaches to individual needs
