Job Summary
Certified Rehabilitation Counselor specializing in Independent Living Skills Training Services (ILST) to assess waiver participants, develop ILST Detailed Plans, and provide hands-on training in ADL/IADL, self-care, medication management, task completion, communication and interpersonal skills, mobility, community transportation, money management, and household maintenance; supports participants in achieving independence in home or community settings through tailored training and problem-solving, with a focus on functional assessments, goal setting, and individualized training plans.
Required Qualifications
- Certification as a Licensed Master Social Worker
- Knowledge of ADL and IADL assessment and training
- Strong interpersonal and communication skills
- Empathy and patience in working with individuals facing disabilities
- Problem-solving skills and the ability to adapt training approaches to individual needs
