Job Summary
As a Service Coordinator with eligibility for Licensed Master Social Worker, you will assist waiver applicants/participants in gaining access to vital services to return to or remain in the community. Responsibilities include collaborating with waiver participants to identify goals, developing individualized service plans (SPs) aligned with those goals, coordinating SP implementation with service providers and stakeholders, regularly reviewing SP effectiveness and making adjustments, empowering participants to advocate for themselves, supporting PPO completion when required, and maintaining up-to-date knowledge of waiver and Medicaid State Plan services. The role emphasizes a voice for participants in decision-making and promoting independence, with duties also involving outreach within Medicaid waiver programs (NHTD/TBI) and related resources. Must hold a Master Social Work degree (LMSW eligible) and demonstrate strong interpersonal, collaborative, empathetic, problem-solving, and client-centered skills.
Required Qualifications
- Licensed Master Social Worker (LMSW) or MSW with licensure eligibility
- Strong interpersonal and communication skills

- Problem-solving skills and commitment to promoting independence
