Job Summary
Licensed Master Social Worker with LMSW licensure to serve waiver participants under PBIS framework. Responsibilities include conducting comprehensive behavioral assessments, developing a holistic structured behavioral treatment plan with measurable goals, training family members, natural supports, and other providers to implement the plan, regularly reassessing effectiveness, and collaborating with health/mental health professionals to align plans with medical diagnosis and disease progression. Requires knowledge of Medicaid waiver programs (NHTD, TBI), and evidence-based behavior-analytic practices; strong communication and collaborative skills to coordinate with a multidisciplinary team in a community-based setting.
Required Qualifications
- Licensure as a Licensed Master Social Worker (LMSW) in accordance with state requirements
- Experience in conducting behavioral assessments and developing treatment plans
- Strong communication and interpersonal skills
- Knowledge of evidence-based practices in behavior analysis
- Ability to work collaboratively with a multidisciplinary team
