Job Summary
Licensed Master Social Worker (LMSW) with a Master’s in Social Work providing Community Integration Counseling (CIC) to waiver participants. Responsibilities include one-on-one counseling to address emotional/psychological challenges related to disability adjustment, providing CIC to individuals and significant others, occasional family or group counseling, upholding client confidentiality, and collaborating with a multidisciplinary team (SCs, ILST, HCSS) to ensure holistic care and integration. Roles may occur in office or in the waiver participant’s home; strong interpersonal skills, empathy, ability to work independently and within a team, and a focus on confidentiality and client-centered care are required.
Required Qualifications
- Master's degree in Social Work
- LMSW licensure in New York State
