Job Summary
Service Coordinator for waiver programs (NHTD/TBI) assisting waiver applicants/participants to access services, developing and coordinating individualized service plans (SPs) with participant input, coordinating SP implementation with providers, reviewing plan effectiveness, empowering self-advocacy, assisting with Plan of Protective Oversight (PPO) where required, and maintaining up-to-date knowledge of waiver/Medicaid State Plan services to connect participants with resources.
