Job Summary
Licensed Master Social Worker needed to provide Independent Living Skills Training Services (ILST) for waiver participants under Medicaid waiver programs (NHTD, TBI). Responsibilities include conducting comprehensive functional assessments of ADL/IADL, developing individualized ILST Detailed Plans with goals and milestones, and delivering hands-on training to improve self-care, medication management, task completion, communication and interpersonal skills. Duties also cover mobility, community transportation, money management, and household maintenance support, addressing maladaptive behaviors, and delivering training in participants’ homes or community settings to maximize learning. Qualifications require certification as a Licensed Master Social Worker with knowledge of ADL/IADL assessment and training, and strong empathy, problem-solving, and adaptive skills for diverse participant needs.
